Participate in a Parent Survey on High School Transition Services

Written by Stephanie Johnson

Dear Parents,

My name is Stephanie Johnson, and I am a doctoral student at Walden University. I am conducting research to obtain the perceptions of parents who have a child with a significant disability. The goal of this research is to determine if parents feel that the high school transition services provided for their child were effective, thus enabling the child to obtain employment or volunteer experiences after exiting high school.

I am asking parents to complete a short online survey containing 37 questions that pertain directly to their child's high school transition services. Each question is multiple choice on a likert-type scale and should take you no longer than 10-15 minutes to complete. It is proposed that the data from this study will be used to improve transition services in the local high schools, while expanding the employment opportunities for young adults with significant disabilities after they exit high school.

Your time and cooperation in this study is greatly appreciated. If you choose to participate in this study, please "click" the hyperlink below and you will be directed to an informed consent page explaining the confidentiality and anonymity of the study. Please note that your participation is completely voluntary and you can exit the survey at any time.

Thank you in advance for your support.
